Doctor Visit Checklist After an Alabama Work Injury

Before the visit

• Write down your main symptoms in simple words.
• List where pain travels, such as neck into arm, back into leg, or shoulder into hand.
• Bring a current medication list.
• Bring prior restrictions, FCE paperwork, or work-status notes.
• Write down what parts of the job you cannot safely do.

During the visit

Be accurate. Do not exaggerate, but do not minimize. Tell the doctor about numbness, weakness, falls, headaches, memory problems, grip problems, sleep problems, and trouble driving or lifting if those problems are real.

After the visit

• Get a copy of the work-status note.
• Save therapy orders, test orders, and referral notes.
• Check whether the doctor changed restrictions.
• Write down what the doctor said about work, surgery, testing, or future treatment.
